National Transition Group
Clinically led by Dr Alok Gupta and Dr Martha Ford-Adams examples of the work that we will be looking into as a group are:
- Why there is limited engagement from adult services due to time restraints
- engaging with young people reaching transition age
- Agreeing standards for transition to adult services and ensuring that they are operationalised
- The national implementation of the Seamless Transition Project funded by the North West Coast Clinical Network (NHS England)
The group will also have the clinical support of Dr Sanjay Gupta and project managed by Margot Carson and Emma Savage
