A Shared Vision for Improving Diabetes Care
The Children and Young People’s National Diabetes Network (CYPNDN) Delivery Plan to 2030 represents a collective ambition to improve diabetes care, experiences and outcomes for children and young people across England and Wales over the next five years.
Developed through extensive collaboration with clinicians, multidisciplinary professionals, regional networks, children, young people and families, the Delivery Plan provides a clear strategic direction and practical roadmap for local, regional and national improvement.
Over the last decade, significant progress has been achieved through partnership, innovation and the dedication of healthcare teams across England and Wales. Access to diabetes technologies has improved, clinical outcomes have advanced, and national collaboration has helped reduce variation in many areas of care.
Despite these successes, important challenges remain. Health inequalities continue to impact outcomes, workforce pressures are increasing, and variation in care still exists across geographical areas and population groups. The delivery plan outlines how CYPNDN and its partners will work together to address these challenges and deliver meaningful improvements for children, young people and their families.

Why This Delivery Plan Matters
Diabetes continues to be one of the most significant long-term health conditions affecting children and young people. As prevalence increases and care pathways become more complex, healthcare systems must continue evolving to ensure that services remain equitable, high quality and responsive to changing needs.
This Delivery Plan has been designed to support action across the healthcare system. It enables providers, regional networks, Integrated Care Boards, Health Boards and partner organisations to align their efforts around a common vision and shared priorities.
More than a strategy document, the Delivery Plan provides a framework for improving outcomes, reducing variation, addressing inequalities and strengthening collaboration between healthcare, education and social care services.

Delivering Better Outcomes Through Partnership
The CYPNDN Delivery Plan aligns with national priorities including the NHS 10-Year Health Plan, GIRFT recommendations, NICE guidance and the National Paediatric Diabetes Audit. By working collaboratively with NHS England, NHS Wales, Integrated Care Boards, Health Boards, professional organisations, charitable partners and families, the network will continue driving improvements that place children and young people at the centre of everything we do.
Delivery Framework
The CYPNDN Delivery Plan to 2030 provides a structured framework for delivering measurable improvements in diabetes care across England and Wales. The framework combines national leadership, local improvement activity, partnership working and the effective use of data to support consistent, high-quality care for every child and young person living with diabetes.
Delivery will be driven through eight dedicated workstreams supported by strong governance arrangements, multidisciplinary leadership, meaningful involvement of children, young people and families, and collaboration with healthcare, education, social care and voluntary sector partners.

The Eight National Workstreams
The Delivery Plan is built around eight workstreams designed to address priority areas for improvement. Each workstream contains specific priorities, actions, measures of success and system enablers that will support local, regional and national delivery.
1. Schools and Social Care
This workstream focuses on ensuring that children and young people with diabetes receive appropriate support across educational settings and social care services, while strengthening safeguarding and multi-agency collaboration.
2. Optimising Diabetes Care and Treatment
Supporting the delivery of consistent, evidence-based diabetes care through increased use of technology, structured education and quality improvement informed by national data.
3. DKA Prevention
Reducing rates of Diabetic Ketoacidosis through earlier diagnosis, improved public and professional awareness, targeted education initiatives and robust use of audit data.
4. Early-Stage Type 1 Diabetes
Supporting earlier identification of Type 1 Diabetes risk, improving pathways into care and strengthening support for children and families through screening, education and service development.
5. Type 2 Diabetes
The Type 2 Diabetes workstream focuses on improving prevention, early identification, treatment and long-term management of Type 2 Diabetes in children and young people. Recognising the increasing complexity of Type 2 Diabetes and its close association with deprivation and health inequalities, this workstream seeks to ensure equitable access to care, education and treatment opportunities.
6. Teenage and Young Adult Care
Supporting young people through adolescence and transition into adult diabetes services remains one of the most important priorities for CYPNDN. This workstream seeks to create consistent, person-centred pathways that support young people through key life transitions while maintaining engagement with services and improving outcomes.
7. Workforce and Speciality Leadership
High-quality diabetes care depends upon a skilled, sustainable and well-supported workforce. This workstream focuses on strengthening leadership, supporting professional development, and ensuring multidisciplinary teams are equipped to deliver safe, evidence-based and innovative care.
8. Type 1 Diabetes and Disordered Eating
This workstream focuses on improving awareness, early identification and support for children and young people experiencing eating difficulties, eating disorders, diabetes distress and related psychological challenges. The workstream aims to promote earlier intervention and strengthen access to specialist support.

Through delivery of the eight workstreams, CYPNDN aims to create meaningful and measurable improvements in outcomes, experiences and quality of life for children and young people living with diabetes.
Working Together to Deliver Change
The success of the Delivery Plan depends on partnership and collaboration across healthcare, education, social care, national organisations, charities and families. CYPNDN will continue working closely with NHS England, NHS Wales, Integrated Care Boards, Health Boards, regional networks and national partners to drive improvement and share learning across the system.
Through collaboration with organisations including DigiBete, Diabetes UK, Breakthrough T1D, the National Paediatric Diabetes Audit and professional bodies, the network will continue to support innovation, co-design resources and ensure that children, young people and families remain at the heart of service development.
