Transitional Care: Supporting Young People Living with Diabetes into Young Adulthood

Transition is more than just the move from paediatric to adult diabetes care—it’s a critical process that ensures young people feel supported, prepared, and empowered at every stage. Effective transitional care goes beyond clinical management; it requires a coordinated approach that considers emotional well-being, self-management skills, and the challenges young people may face as they take increasing responsibility for themselves, and their diabetes. 

The goal is to equip them with the confidence and skills they need to stand on their own two feet by the time they reach 25—the age at which brain development is largely complete, decision-making abilities are fully matured, and young people are expected to manage their health and responsibilities independently. Our services must reflect this understanding, ensuring they are flexible, responsive, and tailored to meet the evolving needs of young people throughout this journey.

Gofal Pontio: Cefnogi Pobl Ifanc Sy’n Byw â Diabetes i Oedolion Ifanc

Mae gofal pontio yn fwy na dim ond symud o ofal diabetes pediatrig i ofal oedolion—mae’n broses hanfodol sy’n sicrhau bod pobl ifanc yn teimlo eu bod yn cael eu cefnogi, eu paratoi a’u grymuso ar bob cam. Mae gofal pontio effeithiol yn mynd y tu hwnt i reolaeth glinigol; mae’n gofyn am ddull cydgysylltiedig sy’n ystyried llesiant emosiynol, sgiliau hunan-reoli, a’r heriau y gall pobl ifanc eu hwynebu wrth iddynt gymryd mwy o gyfrifoldeb dros eu hunain, a’i diabetes.

Y nod yw eu harfogi â’r hyder a’r sgiliau sydd eu hangen arnynt i sefyll ar eu traed eu hunain erbyn iddynt gyrraedd 25 oed—pan fydd datblygiad yr ymennydd wedi’i gwblhau i raddau helaeth, galluoedd gwneud penderfyniadau ar eu hanterth, a’r disgwyliad eu bod yn rheoli eu hiechyd a’u cyfrifoldebau yn annibynnol.
