The Children and Young People’s Wales Diabetes Network (and Brecon Group) brings together health care professionals, third sector organisations and families of children and young people with diabetes. The Network is committed to improving standards of care and outcomes for children and young people with diabetes in Wales as outlined in the Welsh Government’s Diabetes Delivery Plan.

All 7 health boards in Wales are part of the Wales Children and Young People’s Diabetes Network, which is taking forward the work of the Brecon Group, including the national register of paediatric diabetes.

All healthcare professionals in Wales who work with children and young people with diabetes are part of the Network. The Network also supports paediatric teams submitting data to the mandatory National Paediatric Diabetes Audit, and engaging in the Peer Review process.

Mae Rhwydwaith Diabetes Plant a Phobl Ifanc Cymru (a Grŵp Aberhonddu) yn dod â gweithwyr gofal iechyd proffesiynol, sefydliadau trydydd sector a theuluoedd plant a phobl ifanc â diabetes ynghyd. Mae’r Rhwydwaith wedi ymrwymo i wella safonau gofal a chanlyniadau i blant a phobl ifanc â diabetes yng Nghymru fel yr amlinellir yng Nghynllun Cyflawni ar gyfer Diabetes Llywodraeth Cymru.

Mae pob un o’r 7 bwrdd iechyd yng Nghymru yn rhan o Rwydwaith Diabetes Plant a Phobl Ifanc Cymru, sy’n symud ymlaen â gwaith Grŵp Aberhonddu, gan gynnwys y gofrestr genedlaethol o ddiabetes pediatrig.

Mae pob gweithiwr gofal iechyd proffesiynol yng Nghymru sy’n gweithio gyda phlant a phobl ifanc â diabetes yn rhan o’r Rhwydwaith. Mae’r Rhwydwaith hefyd yn cefnogi timau pediatrig sy’n cyflwyno data i’r Archwiliad Diabetes Pediatrig Cenedlaethol gorfodol, ac yn cymryd rhan yn y broses Adolygu Cymheiriaid.
