The Wales Network Referral Pathway for newly diagnosed children and young people was recognised as best practice, winning an award for Diabetes Collaboration Initiative of the Year. The pathway has been audited in Cardiff & Vale University Health Board where it was first piloted and it appears that inappropriate delays in referral have been reduced. The NHS Wales Deputy Chief Medical Officer has asked all health boards in Wales to adopt the Pathway.

In the same category, the All-Wales Standard for People with Diabetes Moving from Peadiatric to Adult Services within NHS Wales was highly commended. This Standard was launched in 2017, after an extensive development programme. It has now been recognised as best practice and health boards are encouraged to continue implementing its recommendations.

The full list of winners were:

Wrexham Maelor Hospital Transition Team – WINNER, Emotional Wellbeing Programmes for People with Diabetes – Children, Young People, And Emerging Adults
