Top Tips for Brecon Group Consent and Registration
Prepare registration paperwork in advance and keep with consent forms etc in newly diagnosed patient pack
If you have the details of a transferring in patient, before their arrival, remember to ask Rhia if they are already on the Register – It may save you filling out a new registration form and consent form!
Talk to families on day one of diagnosis, if possible, and complete as much of the registration form as possible at this time. A lot of information on the family can be obtained by discussing presentation symptoms, HbA1c and family history (family diagnosed with type 1 diabetes).
Use the form as the beginning of the patient and families education
Let families know what they can expect at the beginning, such as upcoming SEREN education, Multi-Disciplinary Team, out-patient clinics
Inform them of the Children and Young Peoples Wales Diabetes Network (and Brecon Group) and the work it does
The best time to complete the registration paperwork is as soon as possible, for example when all blood results are in. When put off, the time soon flies, and a month may go by before you know it, thereby missing our deadline
