A life approach to Diabetes - Issues and Solutions

We invite you to the Collaborative Diabetes Conference.
 

This event is organised by the London Diabetes Clinical Network, Children and Young People’s South East Coast and London Diabetes Network, Healthy London Partnership, South East Clinical Network, Diabetes UK and JDRF.
Aims and Objectives:
We hope that colleagues working in diabetes, whether with children, young people, or adults will work more closely to improve diabetes outcomes, on this day we will particularly focus on transition of children and young people to adult diabetes services.
Our aim is to
·     Share best practice and get teams thinking about how they can replicate initiatives from other areas.
·     Find new ways to address today’s challenges in transitional care.
·     Focus on achieving better diabetes outcomes.
Event Information
Our National Clinical Directors for Diabetes within NHS England will chair this conference.
·     Professor Jonathan Valabhji, National Clinical Director for Obesity and Diabetes
·     Dr Jackie Cornish, National Clinical Director for Children and Young People
The event will bring together up to 200 regional diabetes clinicians, providers and commissioners to focus on improving outcomes through innovation and collaborative working. The event is aimed at specialist diabetes teams, GPs, Diabetes Commissioners and other stakeholders.
